Strange Photos and The Secret Puzzle

You will find some pictures throughout game. If you count things within each Strange Photo (example: the open windows in photo 1 = 6), then count that number across the writing on each one, you will get a letter. It spells out: "YOUVE BEEN HERE FOR TWO DECADES"

Mary's Letter

Mary's letter in the Inventory disappears later in the game, hinting to the fact that it's not the real letter from her.

Inventory's Background

Inventory's background looks like the blurry and distorted version of the video tape you find in the hotel. James doing that thing to his wife.

Missing Wedding Ring

If you look at Jame's left hand (with a freecam mod), you will notice a missing wedding ring. You can find that ring later in a Lost and Found box in the club you go with with Maria.

Body in Backseat

Someone's body is in the backseat of the car in the beginning of the game. Don't look it up if you haven't finished the game, you will know when you finish the game.

Eddie's Van

The Van next to the stairs belongs to Eddie, which you will meet later in the game.

Silent Hill 2: Enhanced Edition Ranch Sign

Silent Hill Ranch sign from Silent Hill 2: Enhanced Edition can be seen inside the barn behind it. The older sign was criticized because of using Comic Sans font. This can be only viewed with a freecam mod.

Angela's Nails

In the beginning of the game at the Graveyard, if you get a close look at Angela's nail with a freecam mod, you can see that they are bitten. Angela is a sexual abuse victim.

Tenebrae

From entrance of the Graveyard, going far right there is a grave after the wall ends with a quote on it from Tenebrae (1982) movie. [Quote on IMDb]

The First Monster Cutscene

The first monster cutscene varies based on the direction from which the player approaches from. [Source]

Endings Books (New Game +)

When you enter the town (the street between Big Jay's and American Cafe in the map), you will see a store called Andy's Book Store. Go inside from store's open window to find three open books referencing game's three endings. (Books are only visible in New Game +)

Wood Side Apartments

Pyramid Head: First Encounter

At the Wood Side Apartments and in the second floor, going inside the green double door in your left, you can see Pyramid Head behind the bars before entering the room 213.

TV Morse Code: Why did you do it James?

The TV in Room 213 of Wood Side Apartments with a dead James on the chair has a Morse code that says "Why did you do it James?" [Source]

Dead Bodies of James

Dead bodies you encounter throughout the game are James's own body.

Mary's Room

The Apt. 201 you enter after solving the coins puzzle has a small room in right after the entrance that's Mary's Room when she was sick. Also the picture on the wall is Mary with James holding her hand.

Bluecreek Apartments

Moving Mannequins Behind Mesh

Mannequins behind the mesh while solving the clock puzzle in Bluecreek Apartments move closer to mesh every time you solve one part of the puzzle.

Angela's Family Photo

In the Bluecreek Apartments and after finding the Minute Hand, when you go to the mirror room in room 109, you can find Angela's family photo on the ground in front of the door.

Pyramid Head might be James

While fighting Pyramid Head for the first time in end of the Bluecreek Apartments, if you take a closer look at his hands you can see some dirts similar to when James puts his hands inside dirty holes. Also in game files it shows that James and Pyramid Head have the same height.

Western South Vale

Maria's Original Outfit

In the motel where you can go up the stairs behind you and break the window next to room 108. Opening the door for Maria and then going through the broken wall to get the note with code on it for the safe, getting back to other side of the broken wall, Maria shows her original dress from Silent Hill 2.

Baldwin Mansion

After saving Maria from a enemy and opening the locked gate in the motel, just in front of you is the Baldwin Mansion. Go inside and then come back out and interact with Maria.

A small conversion between Maria and James will happen which is related to BfaW. Born from a Wish is a DLC content that should be added later to remake. It's about Maria exploring the mansion and meeting Ernest, or rather his voice, behind a door.

Married Man's Stuff in Lost and Found

In the Strip Club with Maria, there is a Lost and Found box with James's stuff in it. [Source]

An empty wallet with a photo of a woman with her face scratched out.
A lost wedding ring.
In a strip club.
Someone didn't want to be seen going in.
Someone was angry and blew all their money.
Someone didn't want anyone to know they were married.

Nurse Uniform

In the Strip Club with Maria, there is a nurse costume on one of the chairs. It's a secret costume that can be obtained in Silent Hill 4: The Room.

James's Film in Theater

In the Western South Vale when you go inside the theater after climbing inside and leaving Maria outside, and in the second salon after meeting Eddie, Laura will turn on the projector and if you keep looking at the white screen, you will see a one second of the film that James does that thing to his wife.

Brookhaven Hospital

Mary in Photo of Brookhaven Hospital Personnel

After opening the second double door from entrance of the Brookhaven Hospital, look at your right to see a photo of hospital's personnel with James's Wife, Mary, in it. (some people said they didn't see Mary in their version of photo, so it might need something to happen to appear)

Guilt on Poster

From entrance of the hospital, keep going to your right to a hallway that leads to the basement to see a readable poster on the wall in your left. If you read the poster, James will remove the word Guilt. Hinting to the fact that (maybe?) he doesn't see himself guilty for what he did.

TV Morse Code

The TVs in Cafeteria and Murses' Lounge at the Brookhaven Hospital have a Morse code that repeats "AGAIN AND". [Source]

Eastern South Vale: Otherworld

Heather's Room from Silent Hill 3

After a cutscene with Angela and getting the key from the ground for the Silent Hill Historical Society, get out of the Rose Park and keep going forward to a motel in the other side of the street. Go to room 106, this is Heather's Room from SH3.

Bloober Team

There is a group picture of BlooberTeam (SH2R Developers Team) in Silent Hill Historical Society. From entrance go right and check the wall behind a locked gate to upstairs.

Labyrinth: Ruined Area

Jame's Aim is Shaking

In the Eddie boss fight in the freezer, Jame's aim will start shaking because of how cold it is or maybe he doesn't want to kill another human!

Lakeview Hotel

Burned Down

The hotel will slowly rot as you progress through the story. The real Lakeview Hotel has been burned down by some fire incident way before James comes back to it.

Se*ual Noises

After entering the Room 205 after defeating the Mandarin and solving the bookshelf, you may hear se*ual noises of James and Mary.
